Definitions of Provider Types as classified by CHC Credentialing

Attending Providers: Primary Care MDs and DOs

  1. Includes NPs and PAs who work directly with PCP physician or group
  2. Must Specify if On-Site or Telehealth

 

Alternate Physician: On Call MD or DO providers for PCP or you were hired directly from Centers Health Care as a specialty provider

  1. Includes NPs and PAs who work with our Alternate Physicians
  2.  Must Specify if On-Site or Telehealth

 

Consulting: All specialty service providers. Psychiatrists, Cardiologists, Pulmonologists, Nephrologists, Wound Care Providers, endocrinologist, urologists, podiatrists, dentists, Ophthalmologists, etc. (Consulting providers do not have access to create orders. All orders given by Consulting Providers are considered recommendations and if approved will be ordered by the Attending Provider for signature.)

  1. Includes NPs and PAs who work directly with consulting MD & DO providers/medical groups
  2. Must Specify if On-Site or Telehealth

 

Psychology Services: LCSW, Psychologists, LMHC, other counseling service providers with licensure

  1. Includes NPs working with psychology services with Psych Based certifications
